Payment

Co-payments are due at the time of the office visit.

Patient insurance cards should be shown to the receptionist so that proper billing is done correctly. FPA will submit a claim for all insurances and bill the patient for any balance.

Hardship cases may contact FPA to discuss a payment plan.
